0166(3)(a): Front door that is used as an emergency exit was locked during childcare hours. 0166(3)(c): Front door was blocked with playpen and other items to prevent a safe exit. 0166(4)(c)(ii)(iii)(lv)(v): Window in the playroom that is used as an emergency exit does not have a place toland outside and the area outside the window is clutter.
